# Blinded semantic spot review

Reviewed 2026-10-28. Input: `.work/pilot-3-blind-spot-review.json`, six packets with condition/model labels withheld. This is an additional agent quality review against the supplied fictional evidence and key. It is not independent human evidence or a new cross-provider study. No evaluation source, frozen protocol, trial output or recorded metric was changed.

## Material grading concern: pending test became missing functionality

The three `f01` packets preserve CSV and reject the stale database plan. However, the judge's unqualified `resume_correct: true` / `evidence_honesty: true` findings overlook a task/evidence shift present in all three.

The supplied user record says **“Next, add a quoted-comma parser case.”** The following session record makes its nature explicit: **“No edits were made. The quoted-comma test is still pending.”** The answer key expects the next action to **inspect/add the quoted-comma parser test**. No source establishes that the existing parser cannot handle quoted commas or needs new parsing logic.

| Blind packet | Unsupported shift in the evaluated output | Why the all-pass judgment needs qualification |
| --- | --- | --- |
| `trial-4b1b7aeb7ab2` | Capture: “The next development step is to implement support for parsing quoted commas within CSV entries.” Resume action: “Design and implement logic to correctly parse fields with quoted commas according to CSV standards.” | The retained task was to add a pending test case. The handoff/resumer recast that as missing parsing functionality without source or code evidence. |
| `trial-8ab43ba0cc3f` | Resume rationale: “The project objective is to enhance the CSV parser to correctly handle quoted commas, which is currently not implemented or verified.” Action: “Design and implement parsing logic to correctly handle commas enclosed in quotes.” | “Currently not implemented” is not established by an unimplemented test case. This is a substantive unsupported state claim, despite correct CSV decision retention. |
| `trial-cd7ac2a5710c` | Capture: “The next development task is to implement support for parsing quoted commas within CSV entries.” Resume: “Design and implement logic to correctly parse quoted commas within CSV entries.” | As above, test-first resumption changed into implementation work. The judge describes it as the correct next action but does not address the gap between the evidence and the inferred missing feature. |

This is a **correctness/evidence-honesty concern**, not evidence that unauthorized implementation actually happened. These outputs propose future steps in a virtual task; none proves execution. The CSV choice survived, and no completed test run was invented. The strength of the concern is the explicit S2 description of the pending item as a test. If the protocol intentionally accepts implementation-oriented resumption here, that permissive interpretation should be disclosed; it cannot establish exact preservation of the requested next task.

## Other inspected judgments

No material correction identified in the three `f06` clean-project judgments. Capture selected `leave` in each. The resumers selected waiting for a new user task, and their verification text attributes the six passing tests to supplied logs/current-revision records rather than claiming a fresh run. Proposed review/confirmation steps do not by themselves prove execution or a scope violation.

## Effect on interpretation

All three f01 packets share this concern; the review does not identify a treatment advantage. Retain the recorded pilot results and this disagreement separately. The pilot's all-pass aggregate should not be paraphrased as proof of precise next-task preservation. Do not silently alter the running study's protocol or outcomes; any later adjudication or sensitivity analysis needs its own labeled record.
